This issue occurs due to overheating and solder failure of the RSX chip (Reality Synthesizer/Graphics Processing Unit). It is caused by excessive heat, combined with the low grade thermal grease used on the Playstation 3, which transfers heat from the CPU and GPU to the heatsink/fan assembly. Over time the solder becomes weak due to the extreme temperature fluctuations and can become bridged (tin whiskers) or crack and lose connection.
